Many Medicare beneficiaries don’t realize the difference between an Annual Wellness Visit and a traditional physical exam. The Annual Wellness Visit is a unique benefit designed specifically to help you stay healthy and identify potential health issues before they become serious problems.
Health Risk Assessment: We’ll review your health history, including past medical conditions, surgeries, hospitalizations, and current health status. This comprehensive review helps identify patterns and risk factors that might affect your future health.
Medication Review: We’ll go through all your medications, including prescriptions, over-the-counter drugs, and supplements. This review helps identify potential drug interactions, unnecessary medications, and opportunities to simplify your medication regimen.
Preventive Care Schedule: Based on your age, gender, and health history, we’ll create a personalized schedule for preventive services like mammograms, colonoscopies, bone density tests, and immunizations. This ensures you’re getting all the screenings Medicare covers.
Cognitive Assessment: We perform a brief evaluation to detect early signs of cognitive impairment, including Alzheimer’s disease and other dementias. Early detection allows for better planning and access to support services.
Depression Screening: Mental health is crucial to overall wellness. We screen for depression and can connect you with appropriate behavioral health resources if needed.
Fall Risk Evaluation: Falls are a leading cause of injury among seniors. We assess your fall risk and provide strategies to improve balance and home safety.
Personalized Prevention Plan: We work with you to create a written plan outlining your health goals, recommended screenings, and lifestyle modifications to improve your health.
Understanding what Medicare covers can be confusing, but the Annual Wellness Visit benefit is straightforward and valuable.
If you have Medicare Part B and have had it for more than 12 months, your Annual Wellness Visit is completely covered. There’s no copayment, no deductible, and no out-of-pocket costs when the visit is coded correctly as an Annual Wellness Visit.
Your first Annual Wellness Visit is called an “Initial Annual Wellness Visit” and includes additional components like a review of your medical and family history. Subsequent yearly visits focus on updating your health status and prevention plan. Both are fully covered by Medicare.
Important: The Annual Wellness Visit doesn’t include a head-to-toe physical exam. If you have new symptoms or health concerns that require examination or treatment during your wellness visit, those services may incur standard Medicare copays. For self-pay patients needing services beyond the wellness visit, our visits start at just $159. We’ll always discuss any additional services and costs before proceeding.
Medicare covers one wellness visit every 12 months.
Rural Iowa presents unique health challenges for seniors, from limited access to specialists to the isolation that can affect both physical and mental health. Annual Wellness Visits address these challenges proactively.
Many serious health conditions, including heart disease, diabetes, and cancer, can be detected early through preventive screenings. Your Annual Wellness Visit ensures you’re receiving all appropriate screenings based on your age and risk factors. Early detection often means simpler, more effective treatment.
The average senior takes multiple medications, increasing the risk of interactions and side effects. During your wellness visit, we review all your medications to ensure they’re still necessary and working effectively. We can often simplify medication regimens, reducing costs and improving compliance.
Your personalized prevention plan isn’t just a document – it’s a roadmap for healthy aging. It includes specific, achievable health goals, recommended lifestyle changes, scheduled preventive services, and resources for maintaining independence. This plan guides your healthcare decisions throughout the year.
For seniors seeing multiple providers, care coordination is essential. Your Annual Wellness Visit helps ensure all your providers are working together effectively. We can identify gaps in care, unnecessary duplications, and opportunities for better coordination.
Depression and anxiety are common but often undiagnosed in seniors. Our screening process identifies these conditions early, allowing for treatment that can dramatically improve quality of life. We connect patients with appropriate mental health resources, including our behavioral health services.
Knowing what to expect helps you prepare for a productive visit. Here’s how we conduct Annual Wellness Visits at QuickVisit.
When you schedule your appointment, we’ll ask you to complete a Health Risk Assessment questionnaire. This can be done at home or with assistance from family members. Bring a complete list of all medications (including dosages), any specialists you see and their contact information, your family health history if available, and questions or health concerns you want to discuss.
Our staff will verify your Medicare information and ensure your visit is properly coded for full coverage. We’ll review the Health Risk Assessment if you haven’t completed it yet.
While not a physical exam, we do check basic measurements including height, weight, blood pressure, and body mass index. These help track changes over time and identify potential health risks.
This is the heart of your visit. Your provider will review your health history and current status, discuss your Health Risk Assessment responses, perform cognitive and depression screenings, evaluate your fall risk and functional ability, review all medications and supplements, discuss preventive services you need, and develop your personalized prevention plan.
Before you leave, we ensure you have your written prevention plan, scheduled appointments for recommended screenings, referrals to specialists if needed, educational materials about health topics discussed, and clear next steps for maintaining your health.
Preparation helps ensure your Annual Wellness Visit is as productive as possible. Here’s how to get ready.
Bring a complete, current list of all medications with dosages and frequency. Include vitamins, supplements, and over-the-counter medications. List all healthcare providers you see, including specialists, with their contact information. Note any hospitalizations or ER visits since your last wellness visit.
Family health history helps identify your risk for certain conditions. Be prepared to discuss parents’ and siblings’ health conditions, age at diagnosis for chronic conditions, cause of death for deceased family members, and any hereditary conditions in your family.
Write down questions and concerns beforehand. Common topics include memory changes you’ve noticed, new symptoms you’re experiencing, preventive services you’re wondering about, lifestyle changes you’re considering, and resources for healthy aging.
You’re welcome to bring a family member or friend to your visit. They can help remember important information, ask additional questions, take notes during the visit, and provide transportation and support.
