If your child hasn’t had a wellness exam in the past year: We can perform an annual wellness exam that includes sports clearance. This comprehensive exam is typically covered by insurance and satisfies sports physical requirements.
If your child already had a wellness exam this year: We offer sports physicals for just $50 – one of the most affordable rates in Iowa.
Each student will be charged separately for their sports physical.
We accept most major insurance plans for wellness exams, including Medicare and Medicaid. For families without insurance or preferring to self-pay, we offer a sliding scale fee discount based on household size and income.
Yes! Bring your school or league forms and we’ll complete them during your visit. We’re familiar with requirements from districts across Iowa.
