Do I need to do anything prior to my appointment?

You may be asked to complete self-assessment questionnaires prior to your visit. If this is needed, our behavioral health coordinator will contact you before your visit.

If you’ve been established with another behavioral health provider prior, please have any documentation or visit notes faxed to us before your visit. You can fax your notes to (888) 440-2805 or email them to behavioralhealth@quickvisituc.com.
